The subscriber is requested to satisfy himself/herself as to the correctness of the statement and to bring any errors, if any, to the attention of this office within one month of the date of issue with the details of the V.N.O., the amount of the voucher, and a copy of the schedule (voucher) under attestation by the DDO concerned.
Failing to do so, the subscriber will be deemed to have accepted the statement of account as accurate and no request for correction/change in the statement of account shall be entertained after one month from the date of issue.
Please quote correct Policy Number in the Schedule (Voucher) and avoid missing credits.
